Nominee must comply with all criteria — please note the requirement that the nominee must continue to participate and be involved in activities at all levels of the Association. A photograph of the nominee is mandatory.  All information must be completed on the form; any other attachments will be discarded.  Only nominations submitted on this form will be considered.
